Bugfix/patient does not have accessible cash account #115

Describe the bug Some patients do not have linked user profiles via mobile or email. It prevents then from making cash payments since there is no User and PatientAccount created for that patient

Impact Some patients can make cash payments throughout their visit without the intervention of an engineer at Serenity

To Reproduce Steps to reproduce the behavior:

  1. Not sure. This may either be happening because of dirty data or phone number changes
