Describe the solution you'd like Create a report via metabase. This report will be tabular and contain the following fields:

1. Name of patient
2. Patient MR number
3. Patient contribution
4. Payer contribution
5. Status (refunded or canceled)
6. Cancelation requested by
7. Cancelation approved by
8. Canceled or refunded by
9. Date canceled

The report will also have the following filters:

1. Patient MR number
2. Date range for cancelation dates (from and to)
3. Status (canceled or refunded)
