bugfix/display patient notes accurately on save #48

Describe the bug When patient notes are added on the patient profile, they not seen until the page is refreshed. It creates the illusion that the note has not been saved

Also for patient's encounters where text have been input into the "Notes" section, it usually appears as "written by" " Dr Chris Dare" regardless of which doctor types it in. This can be seen in the notes tab after choosing to view the patient from the dashboard.

Impact Doctors or nurses may report notes as not saved and tech support team will be overburdened

To Reproduce Steps to reproduce the behavior:

  1. Switch to OPD workspace
  2. Find a patient
  3. View the profile and switch to the notes tab
  4. Add a note
  5. You'll notice that you can't see the note after you create it
  6. Refresh the note
  7. You'll notice that you can now see the note

Expected behavior When a note is saved, it should be displayed immediately
