Finance team needs a report on patient visits booked via slots. It lists all encounters accessing via booking of a slot (via an appointment or) over a given period of time by payment method. This is the same as the "appointment status report" used by the finance team

Describe the solution you'd like
Create a report via metabase.
The report will be tabular and contain the following fields:
mr number
slot id
patient name
practitioner name
patient type
encounter time
encounter date
patient mobile
appointment status
report date
user name (who retrieved the report)?
Filters:
Date range
Patient mr number
** There may be other filters. Will need to speak with Philip or member of finance team to confirm
