ops/set up health hub metrics for chronic dieases #74

Tell us about your request! Health hub team will like to get test results for patients with chronic illnesses sent daily. Tests for chronic illnesses required at this time are:

  1. Fasting blood sugar -> this has been setup and results can be provided
  2. Glycated hemoglobin (HBA1c) -> this has also been set up and results can be provided
  3. Random blood sugar -> couldn't find results under this test. Will need to speak with Dr. Ben and diagnostics team to confirm that this test has been set up

What will be benefit of this feature? Patients can be monitored and managed effectively. Reduces the risk of death and increases provision of better care

Describe the solution you'd like Create a report a. Patient's first, other, last name b. MR number c. Lab requested d. Mobile number e. Date of visit (when requested) f. Doctor that saw them e. Name of test f. Test result value g. Reference range

Describe alternatives you've considered A clear and concise description of any alternative solutions or features you've considered.

Additional context Action points:

  1. Confirm set up of random blood sugar test
  2. Get a sign off from Nana Pokua on sharing the chronic disease data with Lauretta (practitioner requesting for this info). Related issues: #17
