Doctors can gather context and read important notes from other practitioners concerning the patient. This will lead to better delivery of care

Add a new tab on the patient's profile for referrals. There should be permissions for this
For each referral, display:
a. Practitioner making the referral
b. Practitioner patient is being referred to
c. Referral note
