WebForms - Logout issue #145

What steps will reproduce the problem?
1. Start a survey
2. Select the logout link

An error message will be shown. "Authentication not supported. You were never 
logged in.".

In fact I was logged in.  A solution similar to that on the Administration 
screen which allows a user to logout should be implemented.  On the Template 
management screen is a button to manage the logged in user.  When that is 
selected a dialog is shown allowing them to change their password or display 
name.  There is also a logout button.  This logs the user out of apache.
