Make an issue template #60

This will help a lot when people file new issues as this information is rarely all given but greatly helps debugging issues. If you would like to change things/wording @mjdorma feel free to edit directly in this PR.

The contents look great and it covers off most of the initial triage QA AFAICT. 👍

I really need to buy some time to help out on this project again, it has some dags which I'd like to clean up at some point. Thanks for all the good contributions!
