78 damaged the JS media library integration, or it was broken from the start in the following ways:
Setting the featured image and clicking the "PTE" option disables the upload/media tabs such that they don't reappear on subsequent calls to "set featured image".
Also the IRIS search box is appearing on the initial display, this is probably a simple CSS fix
