Default "Campaign Household Mailing List" report should include Household fields

What steps will reproduce the problem?
1. Install Households 3.5
2. Add Mailing list button to Campaign Layout, go to a campaign, and click

What is the expected output? What do you see instead?

EXPECTED:  Report that can be used for a household mail merge.

ACTUAL: Mailing list does not include household fields (address, greetings, 
etc.), They can't be added via customize b/c they're not in the report type.  
To get them, you have to delete the default report, make a new report of type 
"Campaign with Contacts and Households," and then go into the 
npo02__Households_Settings__c object (via System Console) to set report ID to 
null.

SOLUTION:  The default report that ships should be of type Campaign with 
Contacts and Households, and include the relevant household fields.

What version of the packages are you using?
Households: 3.5.7
