What steps will reproduce the problem?
1. When I try to activate the add-in I get the error message outlined above and
nothing ends up on my toolbar
2. When I click "debug" it takes me to the VBA window and it has "Set
ourToolbar = CommandBars (IDS_TOOLBAR_NAME)" highlighted in yellow with a small
yellow arrow pointing at it

What is the expected output? What do you see instead?
Described above, I was expecting the add-in to be added to the toolbar but
instead got this error message
What version of the product are you using? On what operating system? With
what Salesforce.com edition?
Most recent version (1603), windows 7, force.com
Please provide any additional information below.
I've walked through the process with our salesforce implementation team named
app-x but they have no idea what the problem is either. I've attached a
picture of the window after I click "debug" on the initial error message.
