Simplify FilteredTable initialization #151

provide sensible default for defaultColumnState and columnFactory as well as document how exactly a table should be created.

Done in caf7a0c

Earlier issue still persists, i.e., getDefaultColumnInfo still needs to be called upon creation to initialize certain table state.
