mediaobject/caption
probably should be figure/title
(without '.'):
<figure><title>The Building block layers</title><mediaobject>
<imageobject>
<imagedata fileref="Images/BuildingBlocks.PNG" scale="40" scalefit="0"/>
</imageobject>
<textobject>
<phrase>The Building block layers</phrase>
</textobject>
</mediaobject>
</figure>
The DocBook stylesheets make caption
look like an ordinary paragraph, which doesn't help.
Something not right with capitalisation:
Some of the hard-coded references (that should be footnotes or bibliography entries, anyway) lack a trailing space:
These sorts of notes (including in the abstract) should be either footnotes or bibliography items:

Author name does not need to be bold:

However, DocBook does seem to lack a way to indicate the principal author (assuming that that is the reason for the bold).
