Fix rubocop errors redo #162

Well, here we go again. I have revised #160 and made corrections per Duncan's comments. I cherry picked most of the commits from that branch and tried to go into more detail in the commit messages. It ain't perfect and I didn't fix some linting errors in form_builder.rb, but it's better than what it was and it's a start at an improved gem.

@dgmstuart Please review again.

@dgmstuart Going to merge this because I think I resolved all the issues you brought up in #160. If there are more concerns, please open up a new issue.
