tie things together better #26

See if you can tie everything together a bit more. Under each aim, what are the main conclusions/discussion points, at a high level? How de these multiple hypotheses come together to make a few key points worth remembering? We don't have to get into the weeds here like we did in the results...rather we can focus on scientific, clinical, and public health relevance...and each sub-section should start with these key points.

added a more summarizing paragraph at the beginning not as tied together as i would like, but slightly more palatable
