Migration to bootstrap-loader #41

New Plan -- keep 2 repositories. Other one is https://github.com/shakacode/bootstrap-loader/

Major new features:

CSS Modules (indirectly)
Easy to not include jQuery
Support for Bootstrap v4

We're planning on keeping the same github repository, renamed to bootstrap-loader, and changing the npm module name to bootstrap-loader.

[ ] Issue one more version of npm bootstrap-sass-loader and deprecate
[ ] Have a changelog with migration steps
[ ] Ensure docs are sufficient.
