Misalignment in notify report. #309

While preparing to demo Congo for people at today's meeting, I noticed a minor bug: on the Notices Report, the header in grey is out of sync with the actual data presented. In the data presented, column 2 (whatever that is) doesn't have a header - the header that appears is actually the header for column 3. So, the whole thing ends out of whack by the end. It took me a few moments to figure out why the header ends but the data keeps going and why the header toward the end doesn't match the actual data.

The first two column header labels, BadgeID and Event Number, are missing.

(It would be nice if the report had three radio buttons "Normal", "Notice", or "Both" to filter the report.)

This is a dupe, already fixed.
