What steps will reproduce the problem?

What is the expected output? What do you see instead?
Even though screen shots showed how to enter data in a custom made form example 
bank details. I try to find the how to customize the form for data entry but I 
could not find single entry on that topic in user manual.

No forum to discus about this kind of issues

This is a nice piece of work I love it but without form for data entry is no 
use for me my purpose is to collect field data with GPS locations in surveys if 
this is possible you may claim paperless data collection and will have more 
wider user community
