  1. Change text from "New Request" to "Apply Leave".
  2. See if it is possible to use jQuery-ui/any other plugin to implement the form in dialog. Vertical scroll bar doesn't look good, when the form appears.
  3. A date icon in from date and to date field to indicate that the field requires input value from calendar.
  4. Confirmation of the the leave application can come as an intermediate form instead of window's confirm box.
