Closes #8 by pulling from esm.sh like I suggested.

Urls should be a valid GitHub link as we fetch the sha and commit info from it. I would wish the dist on GH could still be preserved, but if that's the decision, we could fetch from the extension dist instead: https://github.com/antfu/textmate-grammars-themes/blob/326db1cd5f95513a45372a458e0cef13a684f50f/sources-themes.ts#L138-L146
