Since we're more of a workshop than a project like SW Carpentry, I think it makes sense to adapt something more like PyCon's or SciPy's.
I like the template policy on the Geek Feminism wiki quite a bit.
Probably makes sense to generate ours from there, using "Shirtable Workshops" in place of "Conference name", and "workshops" in place of "conference".

I made an anonymous CoC violation form using wufoo.com. It is only available on the day of the workshop. When anyone fills it out, both of us will get an email.
I'm using the free account on wufoo, so I can't add you as an admin.
Currently the Code of Conduct link mentions Software Carpentry and points to their CoC (See General Information -> Requirements).
