submitting-patches page - add change-id hook reference

1. What URL does this issue most closely relate to?

2. Please describe the issue in full.
The only reference to Change-Id I see on [1] is under the 'Incorporating 
Comments' section, nothing about setting up the hook to auto-populate the 
commit message [2].
Currently a new user would have to search this out which assumes the gerrit 
documentation was read. I think a command line for this should be added prior 
or within 'Upload your change' for convenience.
