Top level TOC entry seems to have entry.level == 2 #12

To have the numbers appear on the top level divisions in the sample article, line 50 and line 69 of toc/index.tst have to assume the entry.level is 2.

Seems like it should be 1.

The code does what is expected, but it is confusing how level is being defined.

I think that is because the top-level is now the whole "article" which may be entry level 1? I spoke with Rob about redoing the layout of the doc-manifest.xml so that things can be more consistent.
