opening this PR to capture fixes per #39 and other misc cleanup to the package and code base

I think these last pushes should do it. You'll have to reinstall with devtools::install(build_vignettes=TRUE) or devtools::install_github("signaturescience/focustools", build_vignettes = FALSE) to get the vignette. The vignette is also present in an Articles index under "Getting Started" in the pkgdown site.

I removed all the Usage stuff in the README that's now better described in the vignette. Note also that once made public the README could directly link to the vignette, presumably at https://signaturescience.github.io/focustools/articles/focustools.html. I didn't create this link yet, avoiding R CMD CHECK issues with a 404.
