This consolidates CHANGELOG.md and 'Release Notes.txt' so only CHANGELOG.md needs to be maintained.
The Inno installer now copies CHANGELOG.md into the 'Release Notes.txt' file that's included in distribution.
I kept the existing .txt extension since I doubt most users' Windows will know how to launch a MarkDown file.
