Feature request: Table of contents #10

The standard ToC in SB doesn't work right when published. It would be nice to have the ability to provide a ToC for large/complex notes.

Would tie in nicely with #9

Would you want this on all pages or some, and how would you decide?

I definitely wouldn't want this on every page. Preferably a tag in the frontmatter or a page tag, personally, but allowing a selection criteria in the settings would be suitable (e.g. be able to set my own "where" clause to decide)
