Selector Icon (a cog icon) is missing #68

Hi guys,

First thank you for making this plugin, it's quite handy for serving video files of various resolutions.

I have a project hosted here, if you scroll down to the last video in Chrome Browser, you can see there is a button to the right of the control bar, that has no icon, but when you hover it, you see a list of stream options.

The source code for the player component you see on that page is here. Let me know if you would like to have more information, thanks!

You did not include the css styles in your packaging.

I drafted a PR to add some docs of using import in README.
