Add changelog template #37

This template is used by Cow to populate fresh changelogs. I've written it to roughly match the format we've been following in recent releases.

@Cheddam could you add the new cow config prop to https://github.com/silverstripe/cow/blob/master/cow.schema.json and https://github.com/silverstripe/cow/blob/master/cow-schema.md so it will get validated during CI builds?
