NEW Disable MFA via environment var #420

The YAML configuration is a bit too clunky. Raised the visibility of this (extremely common) need for devs in the README, rather than hiding it away in a "local development" section.

The build failure is unrelated, it also fails on 4 branch

Yep, was an issue with Behat running against PHP 7.4 which we've just resolved. Merging as-is.
