For #9.

Still TODO:

[x] Include script used to convert the database to the new format (see comments on https://github.com/simonw/sfms-history/issues/9#issuecomment-1178269071)
[x] Upload new database somewhere it can be deployed to Vercel
[x] Clean up that index.py module a bit - probably use register_routes instead
[ ] Breadcrumbs on page that link to their document
[ ] Next / prev / page number links
[x] Update search logic to search new table
