Introduce UPGRADING.md #165

See #164 for a motivation for this PR.

As a start, I've added an entry for #120 for the upcoming 4.0 release. Please feel free to add feedback for additional BC breaking changes that need to be in this file!

I also updated the existing README files and advertised the UPGRADING.md.
