At slots 7 & 8 (8 slot backplane) IC39 keeps RST active. Currently I have to "reset" power source to get the board to work.

@tprzywoz Did you have a chance to debug? @marmeladapk Have you tested this circuit in EEM FMC Carrier? It looks that implementation is pretty much the same, so if we're dealing with a design issue we should observe similar behaviour there.
