Does plash-cli require Plash.app be installed or is this self-contained?
Can it be run in the background using command line?
If so is it possible choose the URL and the monitor in which that URL displayed?
And, assuming any of the above is possible, could a command line approach be used to apply Plash URLs to multiple monitors?

This is already documented in the readme. It requires Plash.app.
No, but Plash has a setting to hide the menu bar icon.
You can choose the URL, but there isn't yet a command to view the current URL. This could be added though.
