Press "Create" button in the right upper corner of the screen
Navigate to the "Submit Request" in opened window
Observe the form
EXPECTED RESULT:
"Submit request" form looks tidy

Such labels cross the borders of fields with its values:

Sizes of labels and fields look too unproportional to each other
The style of fields themselves somehow looks arguable but it's difficult to say which solution would be better here
Maybe make the font in these fields a bit smaller? But I am afraid that it can be not so convenient for some people with worse vision though, just somehow I don't like that I can type so little amount of text and then I need to scroll, it just doesn't feel user friendly at all. It would probably be good to increase size of fields "Description" and "Acceptance Criteria" by decreasing the size of such fields

Fields with numbers and dates (Your Balance in Escrow, Initial Funding Amount, Submission Deadline) have too old school elements for increasing / decreasing value. Maybe it's better to just remove them.
Element for date choosing looks strange and not modern at all too
Same goes for date picker window which appears after clicking on this element (this particular window doesn't look so bad but most possible can be improved)
