Bahria Town Hospital Error from DB

What steps will reproduce the problem?
1. Revenue detailed doctor name field missing in by date.
2. Reception Module, when we insert the patient history data, DB holdup and     
to much delay time. 
3. SCL doctor, report manager serial number dual and triple count.
4. In labortory, parameter serial double code number.
5. parameter not inactive, show error
attached files  

What is the expected output? What do you see instead?
