HP Onset hierarchy should have equivalence classes using HsapDv

See screenshot. All except Juvenile done. HsapDv needs to add grouping stage 
for 5-15 years.

Note: we have still to characterize Onset and the axioms that would link it to 
phenotype. The relation used currently treats it like a continuant (e.g. the 
quality), but this could be changed if we characterize as a process (e.g. the 
event or process of the phenotype or disease starting)
