Google checkout button does not render

What steps will reproduce the problem?
1. View Web Invoice
2.
3.

What is the expected output? What do you see instead?
Looking at my source code, the form tag does not close properly.
but here is the look on the front end.....

What version of the product are you using? On what operating system?
I think it's the newest version.

I think the problem was with my theme. I was using the Ultimatum theme. Once I 
installed a different theme or switched back to WP twentyten or twentyeleven, 
the problem went away. Unfortunately, I did not get support from Mojanith, nor 
from Ultimatum.
