Remove reference to OrganizationAccountAccessRole in init.tf #183

The reference to OrganizationAccountAccessRole in terraform/bootstrap/init.tf implies a deployment with multiple AWS accounts. This is not a safe default assumption. Additionally, there is no documentation about this role in the generated project.
