Revise the documentation and integrate a section describing more technical details about the extension workflow (post-endorsement): e.g.

how to name and structure the repository
how to structure the documentation
how to produce the new JSON-LD context
how to extend the API specs

In 3ef6658 I cut some part from the motivations and included post-endorsement details
