bugs in CSV reports #22

CSV reports cannot currently describe a round where surplus votes are
transferred and candidates are eliminated.  The CSV reports for MeekNZSTV
are thus not correct.  

For all recursive methods, the CSV reports don't list all the candidates
whose surplus votes are transferred in a given round.

I'm marking this as closed.  

The structure of the CSV report does not allow for surplus transfers and 
exclusions
in the same round so we can't fix this.

Recursive methods now properly generate CSV reports.
