Without any of the info from the form filled in, I am only guessing... have you tried using the latest version from http://enroll.reicast.com/? This was an issue a couple weeks back that was resolved.

@dhanial can you please amend this and add the version of reicast? We ill re-open the investigation then. You can see the version in the About section. As the form says, the fields should be completed :)

@AbandonedCart maybe we should make the version visible just also the side bar?
