Add documentation properties for top-level concepts #16

Resolves #13 (at least for now. There are still 79 second-level concepts without skos:definition and skos:scopeNote that will be added later.)

I know also adjusted the namespace already (see #14) and will configure w3id as soon as this is up and running on the test system.
