Apologies for review delay; hoping to get https://github.com/sky-uk/cqlmigrate/issues/96 in before this, since it's a much requested feature and we don't want to immediately combine upgrade paths for clients with driver upgrade. Hoping its only a few days delay

Rebased on top of latest master; original branch pushed to update-cassandra-driver-4.x-with-simulacron-ORIGINAL. Comparing this branch with that shows only whitespace changes at time of push.
