This PR attempts to update the PR template to correspond with the new Contributing Guidelines. The template now simply guides people to:

reference the original issue used to discuss the new Tech Radar proposal
provide a link to the Thoughtworks Radar Tool with the PR's branch as the data source, as per the README testing guidelines.
