Missing headings, text and styles in Bible documents #67

I have compared out with BibleTime (Linux platform) and there are some issues 
in rendering when using translations created from IMP files using GBF notation. 

Some of issues can be tested using KJV-format and I'll can provide you some 
copyrighted (testing stage) translations, if you'll needed them to testing. (I 
suppose, you have my contact information, because I'm one of the translators of 
and-bible (English - Finnish translator).

Here are the issues:

1) Text / headings before first verse of chapter can't be shown. They are put 
between $$$book $chapter:0 tag and first verse tag in IMP file. Text includes 
both headings and introduction to chapter.

2) <FB>...<Fb> (bold) text is not rendered as bold but standard text.

3) <FI>...<Fi> (italics) text is not rendered as italics but standard text.

4) It isn't possible to navigate in documents which consist only from aporypha 
(books written in time between old and new testament, which are included in 
original LXX).

The last point is only 'a nice to have' feature, but I assume that points 1 
through 3 are doable features, which would be very nice to have.

Hi Jori,

1) is a Jsword processing issue that may be rectified in the future - I think 
DM said he would look at this.
2) & 3) I have added some processing for <hi type='bold'> and <hi 
type='italic'> and all other standard hi types too, and also modified the 
processing of <p> a little.
4) this is related to Alternate Versification and is not currently supported.

I will test the changes for 2) and 3) for a couple of days and if okay will do 
a second beta build next week for you to look at.
