For Multnomah, swap SID with OTN #388

In Multnomah County, they have SIDs (state IDs) instead of OTNs. POs have requested that we remove the OTN input and include a SID input when they add a new client.

Not sure what this would do to the data structure, so let me know how easy / difficult it would be to deploy this. If it's easy to do, I know POs would appreciate it!
