This fixes Fred's problem with the pgpass.conf file not being found, and more.
Actually it was the directory that we had neglected to create if it wasn't there, so that now happens.
As well as that I changed the way we set up the list of alter scripts in the Postgres version, and how we capture the user details in the SQL Server version, as well as removing -- or at least reducing -- the output buffering for both.
