Add support for ICD9 #98

Allow ICD9 as an option for classifying diseases.

Be aware of the fact that the CDC need to be able to upload datasets that have already been selected by disease type, and that do not include a disease code. A possible approach is to have ICD9/ICD10/Other as the classification types.
