I updated the docs in qualifier, and some of those changes should be represented back in this repo.

This is broken into separate commits for easier reviewing.

More date fields
Add DiagnosticReport Lab profile (and tweak DiagnosticReport Note profile to only look at non-Labs)
Rename c_term_coverage -> c_system_use
Rename q_term_use -> q_system_use
