QA: Verify coverage of US Core profile fields in core tables #181

Using https://hl7.org/fhir/us/core/STU4/artifacts.html, we should verify that the core tables include the required core fields, and if they do not, add them going forward, or create issues to cover harder to cover use cases (i.e. new complex FHIR object types).
